In order to complete your retail application you will need to download and complete the Universal Sales Tax Certificate (G-495) form: Download Tax Exemption Form Here
Please email the completed Universal Sales Tax Certificate (G-495) form to customerservice@ejoistdistribution.com or fax it to 586-803-3415.
After submitted this webform and receiving your Universal Sales Tax Certificate form we will notify you via email once that account has been enabled to purchase Amsoil.
